If you haven't seen the documentary Long Strange Trip, go see it. It's great.
The film features a lot of amazing art and music. The credits only list a fraction of the artwork. The soundtrack album only features a fraction of the songs, and some are different versions than what is used in the movie. This is an attempt to fill in the gaps, for people who want to track down the original artwork and song versions.
All rights to the film content are held by Amazon Video.
The title sequence is a montage of 16 different works of art.

Poem by Emily Dickinson, published posthumously in "Poems: Series 1", 1890
Woodcut by Michael Wolgemut for the Nuremberg Chronicle, Germany, 1493
Lithograph of a watercolor by Hieronymus Hess by C. Danzer, from The Dance of Death at Basel, Switzerland, 1839-1841 (Hess' copy is based on a 1773 copy by Emanuel Büchel, as the original mural was destroyed in 1805)
Lyric from Truckin', Robert Hunter, American, 1970
Painting by Hans Baldung Grien, Germany, 1510
Copperplate engraving on page 7 of Anatomia per Uso et Intelligenza del Disegno, by anatomist Bernardino Genga and artist Charles Errard, published by Domenico De Rossi, Rome Italy, 1691
Multiple variants exist with slight differences, the more common reverse image is attributed to artist Antoine Dieu and engraver Johann Theodor De Bry, France, 1593
Engraving by Sebald Beham, a reverse copy of the original engraving by his brother Barthel Beham, Germany, 1548
Table 87 from Anatomia Humani Corporis, published by Govert Bidloo from drawings by Gerard de Lariesse, Dutch, 1685

Angelo Jank, published in the German magazine Jugend in 1897
One of 38 woodcuts from the Heidelberg Totentanz, printed by Heinrich Knoblochtzer, Germany, 1488
Multiple variants exist, and attribution is a matter of debate, but one of the two copies in the British Museum is signed by Dutch engraver Egbert van Panderen, as is the Rijksmuseum copy, though that credits "Joannes Bernadinus S." (Giovanni Bernardino Siciliano?) as the original illustrator, Dutch, circa ~1610
The 'death' card from the Ancient Tarot of Lombardy, published by Ferdinando Gumppenberg, Milano Italy, 1810
The 'death' card from the modern recreation of the Tarot Of Marsailles designed by Paul Marteau circa 1930 and printed by BP Grimaud, based on the 15th century original
One of several paintings by Hans Baldung Grien known by the same name, Germany, 1518-20
Bonus Painting: Late in "Act I", when the band changes from the Warlocks to The Grateful Dead, they reuse several of those and also include Portrait of Hieronymus Tschekkenbürlin with Death, Basler Meister, 1487.
This listing is incomplete, but is a far better representation of the song versions used in the documentary than the official soundtrack. The soundtrack only includes a small fraction of the music in the movie, and in some cases uses different performances.
★ - included on the official soundtrack (14 songs)
▲ - included on the Amazon exclusive 3 CD soundtrack (4 songs)
(The soundtrack includes Dark Star 2/14/1970, I Know You Rider 6/21/71, and the studio version of Ripple, which do not seem to be in the movie. The 3 disc version also includes Eyes Of The World 8/6/1974 and Dark Hollow 10/7/1980.)
